The prices above are per examinee and
include one time use of the test booklet, scoring, score reporting,
and records maintenance.
All orders must have a valid
institutional purchase order or institutional
credit card number before they can be filled. For orders
placed through this site, please provide the name
of an authorized individual and telephone number by which billing information
can be verified prior to accepting the
order.
Uses other than those listed
above will be priced according to use and must be
approved in advance by PACAT Incorporated.
All testing materials
remain the property of PACAT Inc. Unused booklets may be retained if
they are to be used prior to the expiration date printed on the front
of the booklet. All materials, used and unused, must be returned
within 30 days of the expiration date. PACAT Inc. does not issue
credits or refunds for unused booklets.
Shipment will be by UPS (Ground)
unless otherwise requested (except Alaska and Hawaii). Shipping and
handling fees must be paid by the ordering institution.
Orders for paper versions must be received at least 10 business days prior to the intended administration date. In the event of
an emergency, please call us to determine whether or not more rapid shipment can be accommodated. Additional
fees will apply to all orders received less than 10 business days prior to
administration. Orders for ACAT Online authentication codes will
not incur a rush processing fee if they are received at least 48 hours
in advance of administration and are to be delivered by email.
Providing an incorrect shipping address or an address to which your institution will not permit direct UPS
deliveries may result in a UPS delivery redirection fee being billed back to your institution with this order.
PACAT fees are invoiced when the
order is shipped. Payment terms are net 30. In the event that this
account is not settled in a timely manner we may, at our option,
declare the entire balance due immediately without notice or demand.
The individual or entity whose name appears on this account must pay
all costs, disbursements and reasonable attorneys' fees incurred by us
in legal proceedings to collect and enforce the debt on this account
in the event of delinquency. For orders placed by institutional
employees without appropriate institutional authorization, these terms
shall apply to the employee as well as to the institution. We reserve
the right to decline further orders for testing services until such
times as accounts have been satisfactorily settled.
